Midwest Health Services, Inc. is seeking a Day Services Program Coordinator to join our team.

This role focuses on collaborative support for individuals with ID/DD, advocating for their rights, and coordinating home and community services to promote independence and meaningful life experiences. The ideal candidate is self-motivated, highly organized, and an effective communicator who thrives in a team-driven environment, helping individuals achieve their goals with guardians and families

Responsibilities

• Works in a collaborative team process supporting individuals with ID/DD.
• Advocates for the rights of those with Intellectual/Developmental Disabilities (ID/DD).
• Supports a team serving individuals with ID/DD in home and community settings.
• Participates in service plan development with the team.
• Engages guardians, family members, and friends to support individuals' hopes and dreams.
• Encourages community integration, self-advocacy, and independence.

Your Responsibilities Will Include, but are not limited to

• Works in a collaborative team process supporting individuals with ID/DD
• Advocating for the rights of those with Intellectual/Developmental Disabilities (ID/DD)
• Supporting a team of personnel serving individuals with ID/DD in their home and community
• Participate as a team member in support of individuals with ID/DD in service plan development
• Works with guardians, family members, friends, in support of the individuals' hopes, desires, dreams
• Encourages community integration, self-advocacy, and independence of those we serve

• Intellectual/Developmental Disabilities field as direct support/management
• Long Term Care - Assisted Living, Nursing Home
• Home Health Care
• Settings requiring teamwork - team leader, crew leaders
