Job summary

Sunrise Community, Inc. is seeking a Direct Support professional to provide personalized assistance to individuals with developmental disabilities in a community setting in Florida. You will develop, implement and monitor individual program plans, and coordinate training to meet identified needs.

Strong documentation skills are required to ensure plans meet regulatory requirements. A valid driver’s license with endorsements and at least three years of driving experience is essential to support

Qualifications

• Must pass pre-employment background and drug screenings.
• Must have a valid driver's license with endorsements and have a minimum of 3 years driving experience.
• Must possess advanced computer skills.
• High School Diploma or equivalent; Bachelor’s degree preferred.
• Experience in intellectual/developmental disabilities preferred.
• Minimum one year of writing program plans, case notes, or related documentation.

Responsibilities

• Develop, implement and coordinate individual’s program plans.
• Develop, implement and monitor individualized training programs to meet identified priority training needs.
• Ensure all plans are person centered and reflect the individual’s hopes and dreams through active participation wherever possible.
• Ensure documentation is accurate and complete according to regulatory requirements.
• Communicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressed.
• Performs other duties as assigned.
