Job summary

VitalCaring Group seeks a PRN Weekend Home Health Registered Nurse to lead Starts of Care and weekend patient transitions in the home. You’ll work autonomously with a supportive clinical team to deliver high-quality, compassionate care from day one.

You will conduct in-home assessments, develop care plans, manage medications, coordinate with physicians and therapists, educate families, and document care in the EMR. Licensure and CPR are required.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program; BSN preferred.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care).
• Strong clinical judgment and independent work capability.
• Excellent communication and patient-centered approach.
• Ability to manage caseload and prioritize effectively.
• Proficiency with EMR systems and reliable transportation.

Responsibilities

• Conduct in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including medication management and treatments.
• Serve as case manager, coordinating across therapists, aides, and physicians.
• Monitor progress and adjust care based on changing conditions.

• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR and ensure quality records.
• Supervise Home Health Aides and participate in on-call rotation.

Role Overview

As a Home Health RN, you are the clinical lead for your patients - managing the plan of care, coordinating across disciplines, and driving outcomes from admission through discharge.

You’ll combine advanced assessment skills, care planning, and patient education to reduce hospitalizations, improve recovery, and support long-term independence at home.

What You’ll Do

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
• Develop and manage individualized care plans in collaboration with physicians and care teams
• Deliver skilled nursing care, including medication management, treatments, and interventions
• Serve as the case manager, coordinating care across therapists, aides, and physicians
• Monitor patient progress and make clinical decisions based on changing conditions
• Educate patients and families to support disease management and independence
• Coordinate care transitions and discharge planning to reduce readmissions
• Maintain timely, accurate documentation in the EMR
• Supervise and support Home Health Aides as needed
• Participate in on-call rotation as appropriate

What You Bring

• Active RN license (or compact eligibility) + CPR certification
