Job summary

Enhance Rehabilitation is seeking a qualified Speech Language Pathologist to join our dynamic team. You will evaluate and treat patients by following an individualized plan of care after an initial evaluation, in a SNF setting, ensuring high-quality patient outcomes.

You will work with a treatment team, communicate effectively with patients and families, and apply standardized assessments and swallowing evaluations to monitor progress and adjust plans as needed.

Qualifications

• Experience with standardized speech and language assessments.

In this role, you’ll evaluate and treat patients by following an individualized plan of care following an evaluation.

• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.
• Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.
• Experience with administering standardized speech/language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.
