Job summary

Enhabit Home Health & Hospice is seeking a licensed Master of Social Work to join our team in Texas. The role administers social services to patients receiving intermittent professional services and involves teaching patients, families, and the care team to support care at home.

The ideal candidate has a CSWE-accredited MSW, current state licensure, at least one year of health-care social work experience, and comfortable using mobile devices and basic technology as part of daily practice.

Qualifications

• Must have a master's degree in social work from a CSWE-accredited school.
• Must be certified or licensed in the state where they currently practice.
• Must have at least one year of social work experience in a health care setting.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).

Responsibilities

Education and experience, essential

• Must have a masters degree from a school of social work accredited by the council on social work education.
• Must be certified or licensed in the state where they currently practice.

Education and experience, preferred

• Medicare home health or hospice experience is preferred.

Requirements:

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
