Responsibilities

• Conduct assessments of psychosocial status, communicating findings to the RN and interdisciplinary team to inform care plans.
• Conduct safety and environment assessments, identifying and developing alternative plans to ensure patient well-being.
• Perform social evaluations and plan interventions to address patient and family needs.
• Provide counseling to patients and families, focusing on stress management and coping with difficulties related to the illness.
• Maintain accurate clinical records and enter data into the EMR at the bedside.
• Offer information and referral services to patients and families regarding practical and environmental needs.
• Serve as a liaison between patients, families, and community agencies to facilitate access to services.
• Develop and maintain contacts with public and private agencies to enhance resources available for patients and staff.

Job Summary:

• Serve as a liaison between patients, families, and community agencies to facilitate access to services

• Develop and maintain contacts with public and private agencies to enhance resources available for patients and staff

Requirements:
