Job summary

HealthPro Heritage in Columbus, IN is seeking a PRN Speech Language Pathologist to provide outpatient services in an Assisted Living/Memory Care setting. You will collaborate with facility staff to deliver high-quality therapeutic care.

Qualifications

• Education: Degree in Speech-Language Pathology from an accredited institution.
• Licensure: Valid state licensure as a Speech Language Pathologist, or license eligible.
• Certification: ASHA CCC-SLP certification required.

Responsibilities

• Patient Assessment: Conduct comprehensive evaluations of speech, language, voice, and swallowing abilities using standardized tests and observations.
• Treatment and Education: Implement evidence-based therapies, monitor progress, and educate patients and families.
• Documentation: Maintain accurate records of evaluations, progress, and treatment plans with compliance.
• Collaboration: Work with healthcare providers and participate in multidisciplinary meetings.
