Job summary

Memorial Hospital Belleville, part of BJC HealthCare, seeks an experienced RN to support care management and discharge planning. You will work with physicians and staff to coordinate patient care and ensure optimal outcomes in a fast-paced hospital setting.

Onsite role with PRN shifts (24 hours per 6 weeks), mostly weekdays, day hours (7:30am–4pm), and occasional Saturday work every other month. BSN preferred; 2–5 years of experience; RN licensure required.

Qualifications

• RN licensed nurse with nursing diploma/associate's degree.
• Bachelor's degree in nursing preferred.
• 2–5 years of nursing care management experience is preferred.

• Coordinate, negotiate, procure and manage patient care to achieve quality outcomes.

Additional Information About the Role

Location: This position will work at Memorial Hospital-Belleville and Memorial Hospital-Shiloh

This position is onsite

Overview

Memorial Hospital Belleville is an acute care hospital offering medical and surgical services plus critical and emergency care including Children’s at Memorial for pediatric emergency care. It provides patients a full complement of diagnostic and treatment services as well as heart and vascular care. Memorial, offering medical and surgical services plus critical care, is an accredited Chest Pain Center with PCI by the Society of Cardiovascular Patient Care and is designated as an Acute Stroke Ready Hospital by the Illinois Department of Public Health. In addition, Memorial Belleville recently was accredited by the American College of Radiology as a designated Lung Cancer Screening Center. Since 2008, it has been designated as a Magnet®-recognized organization for nursing excellence by the American Nurses Credentialing Center.

Memorial Hospital Shiloh, a 94-bed, all-private suite hospital was recognized with Magnet status in 2018 and provides emergency care, labor & delivery, nursery, medical and surgical services plus critical care.

Care Management includes Utilization Management (Utilization Review), Discharge Planning, Social Services, Clinical Outcomes (Care Managers), Discharge Transition Coaches, Nurse Practitioners, and Clinical Documentation Improvement.

Preferred Qualifications

Role Purpose

Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source.

Responsibilities

• Assesses, coordinates, and evaluates each patient's plan of care.
• Establishes, continuously evaluates, and documents an individualized discharge plan.
• Ensures clinical documentation and cost-effective use of hospital resources.
