Job summary

Ambercare Homecare in Las Cruces, NM is seeking a Registered Nurse to join our growing home health team on a PRN basis. You will perform initial patient assessments, develop and adjust care plans, and collaborate with families to meet nursing needs.

The ideal candidate will have an accredited RN program graduate, NM license, CPR, reliable transportation, and a willingness to work in home care settings. ICU/rehab experience is a plus.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed in the state of New Mexico.
• At least 1 year clinical experience; ICU/rehab/med‑surg preferred.
• Home health experience preferred.
• CPR certification required.
• Reliable transportation, valid driver’s license and auto insurance.

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Provide complete physical assessment and history of illnesses.
• Observe and report changes in patient condition; document and notify supervisor.
• Regularly re-evaluate nursing needs and revise the care plan as needed.
• Administer medications and treatments per provider orders.
• Provide health care instructions to patient and family.
• Coordinate patient care with other providers and case manager as needed.
• Perform other duties as assigned.
