Job summary

Life Care Home Health Family in Texas is seeking a Physical Therapy Assistant to implement treatment plans under the supervision of a physical therapist and physician, delivering high-quality care in patients' homes.

You will educate patients and families, document progress, and participate in case conferences while meeting licensure and CPR requirements.

Qualifications

• One year experience as a PT in a clinical care setting.
• Preferred: Home health experience.

Responsibilities

• Treat the patient in accordance with the program designed by the physical therapist and coordinate with other team members.
• Provide education and training to patients and families regarding the therapy plan, home exercise program, home safety, and diagnosis/illness.
• Take measurements related to the patient’s condition; report and coordinate changes to the supervising physical therapist.
• Maintain professionalism consistent with the code of ethics of the APTA and HHP; report breaches of obligation.
• Provide documentation of patient treatment and progress to the physical therapist and Agency.
• Participate in mandatory case conferences and face‑to‑face patient conferences as required by state regulations.
• Complete appropriate documentation in a timely manner to assure compliance with company policy.
• Demonstrate commitment to professional growth by participating in in‑service programs and maintaining/improving competency.
• Meet mandatory continuing education requirements of the Agency/licensing board.

Patient Population

Contact with patients in the home setting; interacts appropriately with all populations.

Essential Functions

Education

• Associate degree from an APTA accredited school of physical therapist assistants.

Licensure / Certification

• Current driver’s license in good standing; responsible for renewal before expiration.
• Reliable transportation and auto liability insurance.
• Current state professional license as a PTA in all applicable states.
• Current CPR certification.

Physical Requirements

• Prolonged standing/walking required.
• Ability to lift/transfer up to 50 lbs.
• Ability to meet deadlines and patient needs, make quick decisions under stressful conditions.
• Meet patient/family individualized psychosocial needs.
• Requires hand‑eye coordination and manual dexterity.

Environmental / Working Conditions

• Works in patients’ homes under various conditions.
• Possible exposure to blood‑borne pathogens, bodily fluids, and infectious diseases.
• Some exposure to unpleasant weather; on‑call emergency duties as needed.
• Ability to travel locally and work a flexible schedule.
