Job summary

Bethel Homes & Eden Meadows in Oshkosh, WI is seeking a passionate Physical Therapist to join our therapy team on a PRN basis. You will screen, evaluate, and modify plans of care under physician supervision, helping residents maximize independence and quality of life in a senior living setting.

The successful candidate holds a Bachelor’s, Master’s, or Doctorate in physical therapy, active state licensure, and strong organizational and communication skills.

Qualifications

• Requires degree in physical therapy from an accredited program.
• Active state licensure required.
• Strong organizational and time management skills.
• Excellent written and verbal communication skills.
• Ability to supervise Physical Therapy Assistants.

Responsibilities

• Screen, evaluate, re-evaluate, and discharge plan; revise plans of care under physician supervision.
• Collaborate with interdisciplinary team to maximize residents' independence and quality of life.
• Provide therapy services across a split-site setting (Bethel Homes and Eden Meadows).
