Job summary

Trinity Rehab is seeking a dedicated Physical Therapist (PT) to join our Albemarle, NC team on a PRN basis. This role offers the chance to work in a Skilled Nursing Facility (SNF) environment where you can help patients improve mobility and independence.

The ideal candidate is licensed in North Carolina with 0-1 year clinical experience, strong communication, and a collaborative mindset, committed to high-quality, patient-centered care.

Qualifications

• Licensed Physical Therapist in the state of North Carolina.
• Strong clinical skills in a SNF or rehab setting preferred.
• Excellent communication and teamwork abilities.

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Implement therapy interventions to improve mobility, strength, and function
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with interdisciplinary team members to ensure quality care
• Educate patients and caregivers on treatment plans and home exercise programs
• Maintain accurate and timely documentation
