Qualifications

• Bachelor's, Master’s or Doctorate degree in physical therapy from an accredited program.
• Active state license required; licensure as established by state and national boards.

Responsibilities

• Screen, evaluate, re-evaluate and discharge plan; revise plan of care as supervised by physician.
• Provide patient care in accordance with FP policies and therapeutic programs.
