Job summary

Premise Health is seeking a PRN Physical Therapist to join our North Chicago Health Center in Illinois. You will provide outpatient PT services to onsite employees and their dependents, collaborating with physicians, NPs, PAs, nurses, and wellness staff in a 1:1 patient care model.

This role offers a Monday–Friday outpatient schedule, opportunities for ongoing professional development, and access to support resources, with a focus on evidence-based practice and patient-centered care.

Qualifications

• Licenses to practice physical therapy in state.
• Current board certification where applicable.
• Degree from accredited physical therapy program.
• CPR certification with hands-on skills per AHA standards.
• Minimum three years outpatient physical therapy experience.

Responsibilities

• Collaborate with Health Center staff (doctors, NPs, PAs, nurses, dietitians, wellness coaches and community providers) to promote health for onsite employees and dependents.
• Assess and develop treatment plans based on current physical therapy standards and patient values.
• Promote direct access to therapy services and exercise at a high skill level.
• Document and report to onsite and community referral sources; participate in Health Center meetings.
• Engage in continuing competence activities such as peer review and rounds.
• Coordinate with client stakeholders to promote employee safety and ergonomics.
• Develop injury prevention and health promotion programs for onsite employees.
