Job summary
Birchwood-Nursing-and-Rehabilitatio is seeking a Per Diem (PRN) Physical Therapist to join our in-house team serving residents with person-centered care. You will assess, plan, and implement therapies to promote recovery and comfort, while documenting progress and coordinating with the rehab team.

Qualifications
- Active Texas license in Physical Therapy and in good standing.
- SNF/Long term care experience preferred.
- Familiarity with Net Health Electronic Documentation System preferred.
Responsibilities
- Consult with patients to assess their physical condition.
- Develop and implement treatment plans and rehab programs.
- Administer therapy treatments per plan and monitor progress.
- Document thoroughly to regulatory standards and care plan.
- Collaborate with interdisciplinary team to ensure quality care.
- Attend meetings as designated by the Director of Rehab.
