Job summary

Continuum Rehab Group provides in-house therapy with Skilled Nursing facilities in Texas, New Mexico, Arizona, Indiana and Missouri. We are seeking licensed physical therapists to join our clinically driven team and deliver high-quality patient care.

Responsibilities include utilizing evidence-based practice, providing skilled therapy, offering patient and family counseling, and supervising therapy staff in accordance with state regulations.

Qualifications

• Must have graduated from an accredited university.
• Must hold a valid license in the appropriate state (in process is acceptable).

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment to patients with physical functioning disorders.
• Provides skilled therapy services to patients needing rehabilitation.
• Provides consultation to patients, families, caregivers and other providers.
• Supervise Physical Therapy Assistants, temporary licensees, students and support personnel as per licensure requirements.
