Job summary

The Physical Therapist (PT) at Villa Health & Rehab in Florida will deliver an individualized program of therapy, including assessment, treatment, and education to maximize patient progress toward functional goals. This PRN position collaborates with a multidisciplinary team in a quiet, caring SNF setting.

Responsibilities include creating care plans, documenting progress, communicating with patients and families, and participating in department operations to maintain high-quality care.

Qualifications

• Current Florida License as a PT (Physical Therapist).
• Current CPR.
• Experience in a Skilled Nursing Facility preferred.

Responsibilities

• Creates, implements, and executes patient treatment plans.
• Communicates with patients, families, referring physicians, and other members of the team.
• Documents patient progress per facility and insurer requirements.
• Develops and enhances clinical and professional skills through knowledge and professional associations.
• Supports and participates in department operations and development.
• Reports any changes in patient status or needs to Director of Rehab and interdisciplinary team as appropriate.
• Supervises Physical Therapy Assistants with the goal of ensuring optimal patient care and achievement of outcomes.
