Job summary

Dover Health is seeking a PRN Physical Therapist to connect with patients and promote physical, personal, and emotional well-being in their homes and care settings. You will collaborate with the Dover Health team to help patients regain independence and self-care capacities.

Responsibilities include comprehensive PT evaluations, individualized care plans, family education, and discharge planning, while supervising PTAs and maintaining regulatory compliance. Travel to service areas is expected.

Qualifications

• Bachelor's/Master’s/Doctorate degree in Physical Therapy.
• Current Physical Therapy license in state(s) of practice.
• Current Basic Life Support/CPR certification.
• Current driver’s license, reliable transportation, and liability auto insurance.

Responsibilities

• Provide direct patient care activities as directed by physician orders including assessment, treatment planning, implementation, and supervision of physical therapy within Practice Standards.
• Perform initial or comprehensive PT evaluation and develop an individualized plan of care with interprofessional team.
• Educate patients and families about managing illness or injury and home care needs.
• Participate in care coordination and discharge planning.
• Promote patient safety, quality, and regulatory compliance.
• Participate in professional development and lifelong learning.
• Supervise the PTAs.
• Stay compliant with safety and PPE requirements.
• Other duties as assigned.
