Job summary

Trilogy Home Healthcare, a Humana company, is hiring dedicated Physical Therapists to join our award-winning team for home health visits in Palm Coast, FL. The position offers PRN/Per Diem scheduling with per-visit pay and opportunities for growth within a stable organization.

You will provide one-on-one PT services, assess mobility and safety, and help patients achieve goals in their home environments. Florida license, active vehicle insurance, and a valid MVR are required.

Qualifications

• Graduate of an accredited PT program.
• Current PT license in Florida.
• Valid Florida driver’s license, active vehicle insurance, and clear MVR.

Responsibilities

• Provide one-on-one physical therapy services in a home health setting.
• Help patients improve mobility, strength, and balance.
• Set and manage care goals that promote recovery and independence.
• Evaluate the home environment for safety and mobility needs.
