Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans.
• Provide rehabilitative, skilled, and medically necessary treatment per regulatory standards.
• Perform procedures as part of rehabilitation plan: manual therapy, ambulation, therapeutic exercises, modalities, devices, wound care.
• Consult and counsel patients, families, caregivers, and other providers.
• Create discharge summaries compliant with regulatory and clinical requirements.
