Job summary

Sena Kean Nursing and Rehabilitation Center in Smethport, PA is seeking a PRN Physical Therapist to join our in-house therapy team. The PT will evaluate, plan, and deliver interventions to residents with mobility limitations, injuries, and functional impairments.

You will collaborate with an interdisciplinary team, document progress, and educate residents and families while maintaining regulatory standards. Flexible PRN scheduling and a supportive, resident-centered culture are offered.

Qualifications

• Doctorate or Bachelor's/Master's degree in Physical Therapy from an accredited program.
• Current Physical Therapist license in the state of practice.
• Prior SNF or similar setting experience preferred.
• Strong assessment, documentation, and clinical reasoning skills.
• Ability to work independently with a flexible PRN schedule.

Responsibilities

• Perform comprehensive PT evaluations to assess strength, balance, mobility, and function.
• Develop and implement individualized treatment plans based on clinical needs.
• Provide gait training, therapeutic exercise, balance activities, and functional mobility work.
• Document evaluations, progress notes, and discharge summaries per standards.
• Collaborate with OT, SLP, nursing, and social services for coordinated care.
• Educate residents and families on safety, mobility, and home exercise programs.
• Attend care plan meetings and make recommendations based on findings.
