Job summary

Logan Health in Polson, MT is seeking a Part-time PRN Physical Therapist to plan and implement individualized treatment programs. The role requires Montana PT licensure (or multi-state compact) and timely licensure within 30 days of hire, with BLS within 7 days of hire.

Strong communication and teamwork are essential for delivering high-quality patient care. You will perform assessments, develop care plans, document per payer guidelines, and participate in team-based rehabilitation efforts.

Qualifications

• Must hold Montana Physical Therapy license or compact license with authorization to practice in Montana.
• Graduate of an accredited Physical Therapy program and obtain Montana license or compact license within 30 days of hire.
• RQI Provider (BLS) within seven days of hire/transfer or before independent patient care.

Responsibilities

• Develops, implements, and revises individualized, goal directed physical therapy through evaluation and ongoing assessment.
• Performs comprehensive assessments reflecting functional, physical, mental, emotional, psychosocial and environmental status.
• Maintains an updated plan of care with goals, interventions, and visit frequency.
• Completes all required documentation per payer guidelines and Montana practice guidelines.
