Qualifications

• Current Texas licensure as Physical Therapist.
• CPR certification preferred.

Responsibilities

• Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.
• Provide supervision for staff Physical Therapy Assistants in delivery of PT plans of care.
• Assist with patient screenings for the need for Physical Therapy.
• Communicate progress and barriers toward PT goals with interdisciplinary team members.
• Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.
• Maintain rehab gym equipment and treatment area with infection prevention.
• Provide and document patient, family, and caregiver training regarding each PT patient’s plan of care and follow-up.

Education

Texas Physical Therapy License
