Job summary

Lake Eustis Healthcare & Rehabilitation is seeking a Physical Therapist (PT) for PRN coverage in Eustis, Florida. The PT will evaluate residents, develop treatment plans, and deliver evidence-based therapy to promote independence and mobility.

The position requires a current Florida PT license and at least 1 year of SNF/long-term care experience. Flexible PRN schedule and a collaborative rehab team support this role.

Qualifications

• Current Florida Physical Therapist license
• 1 year of experience in skilled nursing or long-term care preferred
• Strong clinical, communication, and documentation skills
• Ability to work collaboratively in a team environment

Responsibilities

• Evaluate residents and establish treatment goals and therapy plans
• Implement physical therapy programs and complete documentation accurately and timely
• Apply gait training, ambulation, strengthening, balance, and mobility techniques
• Design, fit, and train residents in adaptive devices, orthotics, and prosthetics
• Collaborate with the interdisciplinary rehab team to ensure effective care plans
• Educate residents and families on mobility progression and goals
• Supervise PTAs and aides per state regulations
• Provide staff education on physical therapy best practices
