Job summary

Continuum Rehab Group in Texas is hiring a Licensed Physical Therapy Assistant to join a team delivering evidence-based rehabilitation across Skilled Nursing facilities. The role offers pay in the range of $38–$43 per hour, with supervision by a licensed Physical Therapist to ensure quality patient care.

The successful candidate will provide skilled therapy, assess patient progress, document outcomes, and collaborate with families and care teams to support recovery and functional goals.

Qualifications

  • Licensed PTA in good standing.
  • Open to therapists at all experience levels.
  • Ability to document progress and communicate with supervising PT.

Responsibilities

  • Utilizes evidence-based practice to support clinical interventions.
  • Provides rehabilitative and medically necessary treatment under supervision of a Physical Therapist.
  • Delivers skilled therapy services to patients needing rehabilitation.
  • Assesses progress and communicates with supervising PT to adjust treatments.
  • Provides counseling to patients and families regarding physical disorders.
  • Supports discharge planning with supervisor.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:
  • Utilizes evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
  • Provides skilled therapy services to patients who need rehabilitation services.
  • Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
  • Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
