• Collaborate with physicians and care teams for discharge planning.
• Maintain documentation according to department standards.

Responsibilities

• Provides direct PT evaluations and re-evaluations, setting goals and discharge plans.
• Uses documentation forms and coordinates with physicians and health care teams.
• Coordinates and prioritizes own schedule in relation to patient treatment.
• Maintains department equipment and CEUs per state requirements.
• Adheres to health regulations and patient care standards.
