Functional Pathways is seeking a Physical Therapist at Hillview Health and Rehab in Elizabethton, TN. The PT will screen, evaluate, re-evaluate, discharge planning and revise plans of care under physician supervision.

Qualifications include degree in physical therapy (Bachelors/Masters/Doctorate), active state license, ability to supervise PT Assistants, and strong organizational and communication skills. This PRN on-site role emphasizes compassionate patient care.

Job Summary

The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.

Qualifications

• Bachelor's, Master’s or Doctorate degree in physical therapy from an accredited program.
• Certification and licensure requirements as established by state and national board associations.
• Active state license required.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.
• Ability to supervise Physical Therapy Assistants.
• Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.
