Job summary

Functional Pathways is seeking a PRN Physical Therapist to join our therapy team at Smith Village in Beverly Area of Chicago, IL. The PT will screen, evaluate, and revise plans of care under physician supervision, and may supervise Physical Therapy Assistants.

The role offers an hourly wage range of $50.00 - $60.00, with duties centered around providing high-quality therapy services to residents in a supportive, community-focused environment.

Qualifications

• Bachelor's, Master’s or Doctorate degree in physical therapy from an accredited program.
• Active state license required.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Possess knowledge of Functional Pathways policies and procedures.

Responsibilities

• Screen, evaluate, re-evaluate and discharge plan as part of the plan of care, under physician supervision.
• Maintain and revise care plans as needed.
• Ability to supervise Physical Therapy Assistants.

Qualifications

Job Summary: The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.

• Bachelor's, Master’s or Doctorate degree in physical therapy from an accredited program.
• Certification and licensure requirements as established by state and national board associations.
• Active state license required.
• Excellent organizational and time management skills
• Excellent written and verbal communication skills
• Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.
• Ability to supervise Physical Therapy Assistants
• Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.
