Job summary

Swope Health is seeking a Pharmacist for a PRN vacation coverage role in Missouri. The pharmacist will focus on medication management, patient education, and staff collaboration, with oversight of pharmaceutical supplies and drug information.

Responsibilities include developing patient drug records, reviewing therapies for safety, and supporting chronic-therapy monitoring programs with nurse and physician collaboration.

Qualifications

• Degree from an accredited college of pharmacy.
• Registered pharmacist in Missouri.
• Experience coordinating pharmacy programs and teaching health professional students.
• Ability to develop and coordinate a comprehensive health care program.
• 10 years experience in a fast-paced pharmacy.
• Manager experience and 340B knowledge preferred.

Responsibilities

• Develop a patient care program through medication management and education.
• Collaborate with healthcare staff to optimize therapy.
• Oversee pharmaceutical supplies and drug information for staff.
• Coordinate chronic-therapy monitoring and referral to clinics as needed.
• Assist in specialty clinics such as birth control and VD services.
• Lead patient education initiatives on drug use and home testing.
