Job summary

Evolve Therapy Services is seeking an Occupational Therapist to provide in-house therapy within a Skilled Nursing Facility in Lawrenceville, NJ. This PRN position requires planning and directing OT services and ensuring high-quality resident care in accordance with standards and regulations.

Responsibilities include screening, evaluations, treatment planning, scheduling, documentation, and collaboration with the interdisciplinary team.

Qualifications

• Must hold a Degree in Occupational Therapy.
• Active OT license in the practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual therapy schedules with other team members.
• Conduct therapy according to the treatment plan and document/bill OT services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement practices.

Education
