Job summary

Enhance Rehabilitation seeks qualified Occupational Therapist candidates to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care.

As part of the team you will collaborate with facility staff, supervise Occupational Therapy Assistants, and document evaluations, progress notes, and discharges to support optimal patient outcomes.

Qualifications

• Qualified Occupational Therapist with ability to evaluate and treat patients in SNF setting.
• Ability to develop plans of care with measurable, functional goals.
• Experience supervising Occupational Therapy Assistants.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
