Job summary

Haven Home Health & Hospice in Missouri seeks a PRN Occupational Therapist to join our home health team. You will provide skilled OT evaluations and treatments in patients' homes, developing individualized plans of care with patients, physicians, and the care team to help regain independence.

Active Missouri OT license in good standing is required, with one year of OT experience preferred for home health. A valid driver’s license and reliable transportation are also needed to visit patients

Qualifications

• Active Missouri Occupational Therapy license in good standing.
• One year of recent OT experience; home health experience preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled OT evaluations and treatments in patients’ homes.
• Develop individualized plans of care in collaboration with the patient, physician, and care team.
• Help patients improve safety, independence, and ability to manage daily activities.
• Complete required assessments and documentation.
