Job summary

Discovery At Home LLC seeks an Occupational Therapist to provide home health and outpatient services across Independent and Assisted Living communities in Florida. The role emphasizes developing individualized plans, educating patients and caregivers, and collaborating with a broader healthcare team to achieve optimal outcomes.

The ideal candidate holds an OT degree from an accredited program and Florida licensure with CPR/AED.

Qualifications

• Requires degree in Occupational Therapy from an accredited program.
• Florida state OT license and CPR/AED certification preferred.
• Home Health experience preferred.
• One-year outpatient experience preferred.
• Strong communication and independent work skills.

Responsibilities

• Conduct initial evaluations to assess patients' needs and develop individualized treatment plans.
• Develop and implement treatment plans based on patient needs and goals.
• Monitor patient progress and adjust plans as necessary.
• Educate patients and caregivers to improve mobility, reduce pain, and prevent injuries.
• Maintain accurate documentation of patient progress and outcomes.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Provide home health and/or outpatient services, including supervisory visits, as needed.

POSITION SUMMARY:

• Conduct initial evaluations to assess patients' needs and develop individualized treatment plans
• Develop and implement appropriate treatment plans based on patient needs and goals
• Monitor patient progress and adjust treatment plans as necessary
• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation of patient progress
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Provide home health and/or outpatient services as needed, including supervisory visits, to ensure optimal outcomes

Qualifications:

• A degree in Occupational Therapy from an accredited program
• A Florida state OT license and current CPR/AED certification
• Home Health Experience preferred
• One-year experience in an outpatient setting preferred
• Ability to work independently and as part of a team
• Strong verbal and written communication skills
• Possess solid problem-solving skills and critical thinking skills
