You will work in a collaborative, multidisciplinary team to provide evidence-based evaluations and interventions for post-acute and long-term residents, develop individualized treatment plans, and maintain compliant documentation.

Qualifications

• Provide evidence-based evaluations and therapeutic interventions for post-acute and long-term residents in a skilled nursing setting.
• Develop individualized treatment plans to support patient recovery and functional independence.
• Collaborate with an interdisciplinary team to deliver comprehensive care.
• Maintain accurate documentation in accordance with regulatory standards.
