Job summary

Mederi Caretenders Home Health in Port St. Lucie, Florida (a part of LHC Group and UnitedHealth Group) is seeking a PRN Occupational Therapist to join our compassionate team.

You will assess and address patients’ functional status, ADLs, and home health needs, delivering evidence-based OT services under a plan of care, with flexibility to support variable schedules. A valid OT license and CPR are required.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team
• Evaluates the patient's functional status, CMS documentation needs, and occupational therapy needs; consults with the physician in developing the therapy plan of care
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests

Skills

Current Occupational Therapy licensure

Current CPR certification

Valid driver's license
