Right at Home Des Moines is seeking a compassionate PRN Occupational Therapist to join our home care team in West Des Moines, IA. In this flexible per-diem role, you’ll help clients regain independence and improve quality of life in their own homes.

The role offers diverse patient care, collaboration with nurses and therapists, and emphasis on ADLs, cognition, and adaptive equipment. Strong communication and documentation skills are essential.

Qualifications

• Valid Iowa Occupational Therapist license in good standing.
• Graduate of an accredited Occupational Therapy program (MOT or OTD preferred).
• NBCOT certification required.
• Home health or home care experience preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication skills and a patient, empathetic demeanor.

Responsibilities

• Conduct comprehensive occupational therapy evaluations and assessments for home care clients.
• Develop and implement individualized treatment plans to improve clients' functional independence.
• Provide therapeutic interventions focused on ADLs, fine motor skills, and cognitive rehabilitation.
• Recommend and train clients and caregivers on adaptive equipment and home modifications.
• Collaborate with interdisciplinary care teams including nurses, physical therapists, and care coordinators.
• Maintain accurate and timely clinical documentation in compliance with state and federal regulations.
• Monitor and report client progress, adjusting care plans as needed.
