Job Summary
The Occupational Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the Physician.
Responsibilities
- Screen all residents in the facility on a quarterly basis to identify those who have experienced declines in their functional abilities and determine if Occupational Therapy intervention is indicated.
- Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.
- Reassess patients and caseload every 30 days to determine need for continued services or discontinuation based on patient progress. Carry out treatment plan as indicated.
- Supervise treatment implementation carried out by Certified Occupational Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated.
- Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.
- Educate staff, family, and caregivers as indicated.
- Identify needs for program development and implement programs according to company policies and procedures.
- Identify equipment needs of Occupational Therapy Dept. and order equipment according to company and facility policy.
- Complete all required documentation according to guidelines specified in Documentation Manual.
- Coordinate in-services for facility staff as need is determined, to ensure safe and effective carryover of Maintenance/Restorative Programs and overall care of the residents.
- Attend all facility required meetings.
- Promote profession within facility, including coordinating special events during National Occupational Therapy Month.
- Attend Continuing Education courses specific to current position.
- Maintain professionalism and always follow all national/state association’s regulations.
- Supervise student affiliates as directed.
Qualifications
- Bachelor's, Master’s or Doctorate degree in occupational therapy from an accredited program.
- Certification and licensure requirements as established by state and national board associations.
- Active state license.
- Excellent organizational and time management skills.
- Excellent written and verbal communication skills.
- Current working knowledge of Functional Pathways policies and procedures.
- Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.
