Formación

• Current, unrestricted Texas LVN license.
• Experience with seniors or geriatric populations preferred.
• Home care or community-based experience preferred.
• Strong communication and interpersonal skills.

Responsabilidades

• Conduct in-home wellness and client check-in visits.
• Build trusting relationships with seniors through conversation, education, and ongoing support.
• Check on clients' general well-being and identify changes or concerns that may need additional attention.
• Educate clients and families on fall prevention, home safety, mobility, nutrition, medication awareness, and healthy aging.
• Help clients and families identify resources that can support aging safely in place.
• Review client needs and communicate observations or concerns to the care team.
• Support clients and families during changes or transitions in care.
• Accompany clients to medical appointments when requested and help them understand follow-up instructions.
• Coordinate with families, caregivers, physicians, home health, hospice, therapy providers, and other healthcare professionals.
