Job summary

Home Care Providers of Texas is seeking a PRN Licensed Vocational Nurse (LVN) in Henderson, TX and surrounding areas. The LVN will provide and document skilled nursing care per the patient care plan and physician orders, while upholding company standards and boundaries.

Essential duties include performing skilled nursing visits, collaborating with the RN Case Manager to update the plan of care, administering treatments, monitoring patient status, and educating families on care procedures and

Qualifications

• Active LVN license in Texas is required.
• Current CPR certification.
• Experience in geriatric home health care.
• Strong organizational skills and self-motivation.
• Compassionate, patient-focused care.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct skilled nursing visits per the patient’s individualized plan of care.
• Collaborate with the RN Case Manager to develop, review, and update the plan of care based on clinical findings.
• Deliver nursing interventions and treatments as outlined in the care plan with compassion and accuracy.
• Monitor and document patient status; report changes to the RN Case Manager and/or physician.
• Educate patients and families about the plan of care, treatments, and self-care techniques.
