Crossover Health is seeking PRN/As Needed Licensed Vocational Nurses for the Purchase Pepsico Clinic in New York. Availability to cover 8-hour shifts Mon-Fri 8am-5pm is required. Recent phlebotomy experience is required.

The nurse will support a patient-centered home, coordinate with specialists, and provide phlebotomy and immunization services under supervision. This fast-paced role emphasizes primary care, prevention, and a strong patient experience; base pay ranges per hour and eligibility

Qualifications

• Licensed Vocational Nurse with at least 3 years of clinical experience.
• Current Basic Life Support (BLS) certification required.

Responsibilities

• Delivers patient care under RN or supervisor oversight.
• Collaborates with the care team to support a patient-centered medical home.
• Provides phlebotomy and immunization services.
• Coordinates care with specialists and facilities.
• Performs other duties as assigned.
