Qualifications

• Licensed Practical Nurse license issued by Ohio Board of Nursing
• Proof of completion of approved course in medication administration
• Possession of IV Therapy certification if applicable to dialysis patients

Responsibilities

• Administer medications including controlled substances
• Observe and document residents’ condition; report to medical team
• Create progress notes, care plans, and monthly summaries
• Provide general nursing and direct care (vital signs, ADLs, mobility)
• Escort residents to medical appointments or activities
• Enforce safety and infection control measures
• Assist STNA with tasks and training as needed
• Attend team meetings and trainings
• Perform other duties as assigned
