Job summary

Healogics, Inc. is seeking an LPN/LVN to provide wound care under RN supervision in a busy outpatient setting. You will assist in vein clinics, participate in intake and discharge, and support case management while following hospital protocols and BLS standards.

The ideal candidate has 2+ years as an LPN/LVN, current licensure, and is comfortable in a fast-paced team environment. Wound care experience is preferred, with opportunity for hyperbaric therapy duties after training.

Qualifications

• Active LPN/LVN license in the state of practice.
• High school diploma or GED required.
• Graduate of an accredited LPN/LVN program.
• 2+ years as LPN/LVN with SNF/LTACH experience preferred.
• Wound care experience preferred.

Responsibilities

• Provide wound care under RN/Provider directions per BLS and hospital protocols.
• Assist in vein clinic after Healogics Vein Clinic training.
• Assist with intake and discharge as needed.
• Support case management under RN guidance per state rules.
• Gather patient information and necessary equipment.
• Report care quality issues to Clinical Nurse Manager.
• Communicate with team about interventions to ensure quality care.
• Perform hyperbaric therapy duties after training when required.
• May perform scribe duties per policy.

Essential Functions/Responsibilities

• Under the direction of the RN/Provider, provides wound-related patient care according to Basic Life Support (BLS) and approved and hospital protocols, Policies & Procedures
• Assist in vein clinic as assigned after Healogics Vein Clinic training completed
• May assist with intake and discharge
• May provide assistance with case management under the direction of RN, in accordance with state regulations
• Gathers patient information
• Gathers necessary equipment and supplies for the RN or provider
• Reports quality of care issues to Clinical Nurse Manager
• Communicates as needed with all team members regarding therapeutic interventions, to ensure quality of care
• Performs hyperbaric oxygen therapy related duties, as needed, after completing the required Healogics hyperbaric training
• May perform hyperbaric safety director duties after completing the required Healogics hyperbaric safety director training
• May function as a Documentation Assistant (scribe) in accordance with Healogics policy
• Performs other duties as delegated

Required Education, Experience and Credentials

• High School Diploma or General Education Development (GED)
• Graduate from an accredited practical nursing program/state approved educational program
• Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of practice - to maintain throughout the duration of employment in the position
• Two (2) or more years’ experience as LPN/LVN, to include one (1) or more years in SNF or LTACH setting
• Wound care experience preferred

The hourly rate for this position generally ranges between $23.30-$30.69 Per Hour This range is an estimate, based on potential employee qualifications: education, experience, geography as well as operational needs and other considerations permitted by law.
