Collage Home and Community Rehabilitation Partners is seeking an experienced Licensed Clinical Social Worker (LCSW) for PRN roles in Irvine, CA and surrounding areas. You will join interdisciplinary teams delivering patient-centered care in home and community settings.

The position offers an hourly wage of $53.00 to $67.00, with flexible PRN scheduling to support patients through the duration of their stay. Experience in neurological rehabilitation is a plus.

Qualificações

• Master's degree in Social Work and state licensure as an LCSW are required.
• Excellent planning, organization, writing, and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must.

Responsabilidades

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability.
• Coordinate care with families and professionals to support patient goals in home and community settings.
