Collage Home and Community Rehabilitation Partners in Beaumont, CA is seeking experienced Licensed Clinical Social Workers for PRN roles to support patients in their homes and communities. Interdisciplinary teams develop individualized treatment plans and deliver care in patients' homes, addressing neurological and complex orthopedic needs.

Ideal candidates hold a Master's in Social Work and an LCSW license, with strong planning, written and verbal communication skills.
