Job summary

Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in Encinitas, CA and surrounding areas, with PRN positions available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities.

Qualifications

• Master's degree in Social Work and LCSW licensure required.
• Excellent planning, organization, writing and time management skills required.
• Ability to communicate with families, professionals and co-workers.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
