Job summary

Matrix Medical Network seeks a Nurse Practitioner or Physician Assistant (PRN) to perform comprehensive in-home health assessments for health plan members. In this PRN role, you’ll complete detailed histories, physical exams, and risk assessments, identifying care gaps and social determinants that impact health.

You will document findings in EMR systems, educate patients and caregivers, and collaborate with physicians and health plans to improve outcomes.

Qualifications

• Active NP or PA license in Maine.
• Experience delivering in-home health assessments is preferred.
• Ability to perform comprehensive histories, exams and risk assessments.
