Job summary

Matrix Medical Network is seeking a Nurse Practitioner or Physician Assistant (PRN) to perform comprehensive in-home health assessments for health plan members. You will conduct histories, physical exams, and risk assessments, and document findings in EMR systems.

You will educate patients and caregivers while collaborating with physicians and health plans to improve outcomes. This flexible, field-based role supports creative care delivery and continuous learning within a collaborative team

Qualifications

• Current NP or PA license in the United States.
• Experience conducting comprehensive in-home assessments is preferred.
• Knowledge of social determinants of health and care gaps.

Responsibilities

• Perform comprehensive in-home health assessments for health plan members.
• Complete histories, physical exams, and risk assessments.
• Document findings accurately in EMR systems.
• Educate patients and caregivers and coordinate with physicians and health plans.
