Riverside Regional Hospice Williamsburg is seeking a Home Health Aide to provide personal care, comfort, and environmental services to hospice patients in the Williamsburg area.

Under the supervision of a Care Manager (RN or PT), duties include assisting with hygiene, ambulation, basic treatments, and reporting changes. CNA certification and CPR/BLS within 30 days are required, along with a valid driver's license.

Qualifications

  • Prior Nurse Aide work experience in home care preferred.
  • CNA certification required in Virginia; CPR/BLS within 30 days.
  • Valid driver's license and reliable transportation required.

Responsibilities

  • Provides personal care, comfort measures, environmental services for hospice patients as assigned.
  • Assists with hygiene, ambulation, and medications that are typically self-administered (per state rules).
  • Reports changes in patient condition to the supervising RN/CSM and supports team training.
  • Maintains a clean, safe home work environment and ensures equipment supplies are stocked.
  • Provides nutritional support as directed and participates in patient comfort measures.

Williamsburg, VirginiaHiring Range$16.80 - $21.83/Hourly Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.OverviewWorks under the supervision of Care Manager (RN or PT) to provide personal care, comfort measures and environmental services for patients (including those needing special precautions) and performs specified treatments. In addition, this position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served. The individual must demonstrate knowledge of the principles of growth and development over the life span and provide the care needed as described according to the departmental policy and procedure. Promotes and maintains safe work environment for staff as well as our customers, incorporating national patient safety initiatives. Must have reliable transportation to perform assigned home care/hospice visits. The Home Health/hospice Aide may be required to work weekends and holidays as census and staffing require.What you will doProvides personal care, comfort measures, environmental services, and specified treatments for a specified number of patients as determined by department standards.Assists with client's personal hygiene needs, ambulation, exercise, and with medications that are ordinarily self-administered (according to state regulations). Reports any changes in the client's condition and /or needs to the assigned supervising Registered Nurse. Provides nutritional support as needed or directed. Provides other supportive tasks as assigned and/or specified by the case manager. Hospice aides performs duties consistent with Hospice Aide training and permitted to be performed under State Law. Ensures availability of resources (equipment, instruments, supplies) necessary to ensure positive patient outcomes. Recognizes emergency situations and responds competently.Ensures availability of resources (equipment, instruments, supplies) necessary to ensure positive patient outcomes.
