Job summary

Four Pillars Hospice is seeking a Hospice Aide to deliver compassionate in-home care under RN supervision. You will support patients and families, assist with transfers and daily activities, and document changes to ensure seamless care coordination.

The role emphasizes infection control, respectful communication, and maintaining a safe, clean home environment while supporting the patient’s nutrition and comfort needs and contributing to overall hospice operations.

Qualifications

  • High School Diploma Preferred.
  • Successful completion of one of the following: Training program/competency evaluation, Competency evaluation program, Nurse aide training/competency evaluation program approved by the state and listed in good standing on the state nurse aide registry; State Licensure program.
  • Must have a current driver's license.
  • At least one year experience preferred as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long-term care facility or completion of Certified Nursing Assistant or Certified Home Health Aide Program.
  • Must be able to read, write, and verbally report clinical information to patients, representatives, and care givers, as well as to other hospice staff.
  • Demonstrates interest in the welfare of the ill and elderly.
  • Reliable transportation and valid auto liability insurance.

Responsibilities

  • Provide direct patient care to patient under direction of the RN and according to the Aide Plan of Care.
  • Provide necessary skills to assist the patient with safe transfers and ambulation per the Hospice policy.
  • Provide necessary skill to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
  • Practice accepted infection control principles. Provide a clean, safe, and comfortable environment.
  • Promote positive, supportive, respectful communication to patient/family and other employees.
  • Provide an environment which promotes respect for patient, privacy, and property.
  • Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.
  • Contribute to the management and efficient operation of the Hospice and demonstrate effective time management skills.
  • Provide skills necessary to perform treatments and procedures according to the Hospice policy.
  • Promotes the Hospice philosophy and administrative policies to ensure quality of care.

Education

The Hospice Aide's primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment; work cooperatively with the patient, and family; and share observations and problems with the supervisor.
