Job summary

Adoration Health in Colorado Springs seeks a Home Health Social Worker to provide 1:1 visits, crisis counseling, and family support in a home health setting. This PRN role offers flexible scheduling and opportunities for ongoing professional growth within a growing company.

You will assess clients, coordinate resources, and develop care plans to restore social and health functioning while supporting families in the community. Strong clinical experience is required.

Qualifications

• Master’s Degree in Social Work required.
• Current and valid LCSW or LMSW license required.
• Ability to perform psycho-social assessments and develop viable care plans.
• Strong communication and interpersonal skills for counseling families and patients.

Responsibilities

• Guide and counsel individuals and families in crisis, including vulnerable populations.
• Assist clients in obtaining government funds, education, or treatment.
• Collect and evaluate medical, psychological, and social information affecting client care.
• Lead individual or group discussions to plan needs and mobilize resources.
• Support clients in modifying attitudes and behaviors to improve functioning.
• Develop a service plan to restore social and health well-being.
